Adverb of frequency
-It is placed before the simple verb
e.g.: I always have coffee for breakfast.
-It is placed after BE
e.g.: She is usually late.
-If there is a modal verb or an auxiliary in the sentence, it is placed between the auxiliary or modal and the verb.
e.g. (modal): I can never understand what she says.
e.g. (auxiliary): I have always done my work properly!
A few adverbs of frequency :
always, sometimes, frequently,usually, occasionally, seldom, rarely, never,hardly ever ...
